Paying in Hong Kong dollars costs you about 3%.
The pataca is pegged to the Hong Kong dollar at roughly 1.03 MOP to 1 HKD — but almost every shop, restaurant and taxi in Macao takes HKD at par, treating one Hong Kong dollar as one pataca. That difference is quietly yours to eat, on every cash transaction, all trip.
It is small on a bowl of noodles and not small across a week. Pay in MOP where you can, or put it on a card and let the network settle it.
Where each currency actually works
“At par” means they take it, but at 1:1 rather than the real rate.

Cash, cards, and phones
Cash
ATMs across the SAR dispense both MOP and HKD — check which one the machine is about to give you before you confirm. Bank counters beat the storefront exchange booths near the ferry terminal and Senado Square almost every time.
Cards
UnionPay is close to universal. Visa and Mastercard are fine at hotels, resorts and mid-range restaurants and patchy at small local places. If a terminal offers to charge you in your home currency, decline — that conversion is worse than your bank’s.
Mobile pay
MPay, Alipay and WeChat Pay are accepted very widely, including on buses. Mainland Alipay and WeChat wallets generally work; the Hong Kong versions are accepted in fewer places. A Macau Pass card is still the simplest thing for buses.
Patacas are hard to change once you are out of Macao — Hong Kong will mostly shrug at them. Spend down your MOP before you go, or convert it at the ferry terminal or airport on the way out. Keep a little HKD back if Hong Kong is your next stop.
